At Focus Fairies Mentoring, we are committed to empowering and inspiring young minds through mentorship, educational programming, and creative expression. As part of our programs, we may capture moments through photographs, video recordings, and audio to document and share the impact of our work.

This form is designed to inform you of your rights and to seek your permission to use recordings of your child(ren) for educational and promotional purposes.

I acknowledge and agree that:

These materials may be shared electronically or in physical form, including via the internet, digital platforms, public presentations, and printed materials.
There are no geographic limitations on where or how these materials may be displayed or distributed.
This release is valid indefinitely unless I provide written notice requesting to revoke consent for future use.

By signing this form:

I waive the right to inspect or approve any photograph, video, or audio recording prior to its use or publication.
I release Focus Fairies Mentoring and its representatives from any claims, demands, or liabilities that may arise in connection with the use of these recordings, including but not limited to claims for libel, defamation, invasion of privacy, or violation of publicity rights.
I waive any right to royalties or compensation arising from the use of my child(ren)’s likeness or voice.

Should I wish to revoke this consent in the future, I understand that:

I must do so in writing by contacting Focus Fairies Mentoring directly.
Revocation will apply only to future materials and cannot apply retroactively to previously published or distributed media.
